UROLOGY：女性血清睾酮水平和尿失禁的关系

骨盆底完整性是压力性尿失禁的重要预测因子。在骨盆底肌肉组织和筋膜上发现了雄激素受体，而睾丸激素管理已被证实可以增加提肛肌肥大，改善啮齿动物模型的应力性失禁。研究人员研究了血清总睾酮水平与女性尿失禁之间的关系。

骨盆底完整性是压力性尿失禁的重要预测因子。在骨盆底肌肉组织和筋膜上发现了雄激素受体，而睾丸激素管理已被证实可以增加提肛肌肥大，改善啮齿动物模型的应力性失禁。研究人员研究了血清总睾酮水平与女性尿失禁之间的关系。

在2012年的NHANES(国家健康和营养检查调查)中，受试人员包括20岁以上的女性，她们接受血清总睾酮的测量，并回答了自我报告的尿失禁问题。采用加权、多变量logistic回归模型，对年龄、体重指数、糖尿病、种族、平价、更年期和静脉穿刺时间等因素进行调整后，发现尿失禁与血清睾酮水平之间的相关性。

共有2321名女性参与分析，其中37.5%有压力尿失禁，29.8%有尿失禁，16.4%有混合尿失禁。血清睾酮水平最低的女性更有可能抱怨压力和混合尿失禁(或1.45，95% CI 1.03-2.12，或1.68，95% CI 1.23-2.22)。血清睾酮水平与尿失禁之间无相关性。

结论
低血清睾酮会增加女性的压力和混合尿失禁的可能性。鉴于骨盆肌肉组织在维持尿道支持和雄激素对骨骼肌的合成作用方面的作用，可以在前瞻性和转译研究中提出并进一步评估这一关系的生理机制。
