烧伤如何清创更有效?急诊紧急清创可以替代手术室清创吗?

急诊科的程序性镇静和镇痛是全身麻醉下手术室清创术的常用替代方法。

本研究旨在评估全麻下早期(<24小时)非切除清创术与烧伤创面再上皮化时间和皮肤移植需求之间的关系。

该研究纳入2013年1月至2019年12月之间17岁以下烧伤面积为全身表面积的5%或更大的患儿进行队列研究。使用生存分析和皮肤移植需求几率的二元逻辑回归来测试再上皮化的时间,以分析手术室早期非切除清创的效果。

总体而言,292名儿童符合资格(男性55.5%)。

在手术室进行全身麻醉下的早期非切除性清创术,与在急诊科进行氯胺酮镇静清创术的儿科患者相比,显著缩短了再上皮化的时间(14天对21天,p=0.029))和需要植皮的几率(OR:6.97(2.14–22.67),p<0.001。

这项研究首次证明,在手术室全麻下早期非切除清创可显著缩短伤口再上皮化时间,并减少儿科烧伤患者随后的植皮需求。该研究分析表明了急诊科用于烧伤清创的氯胺酮程序性镇静和镇痛,并不能有效替代手术室清创。

综上述,在全身麻醉下对儿童急性烧伤进行早期非切除性清创可减少伤口再上皮化时间和植皮要求。有效的非切除清创应在全身麻醉、温水、无菌手术纱布和无皂表面活性剂清洁剂的积极机械清创下实现。
